BAHN Health Home Modernization
Modernizing and supporting the application behind the Bronx Accountable Healthcare Network (BAHN) Health Home, Montefiore's NY State Medicaid care-coordination program.
Highlights
- BAHN is a network of Bronx healthcare and service providers, led by Montefiore, that assigns each enrollee a care manager and a personalized care plan covering medical, behavioral health, housing and social services.
- Worked with NYS Department of Health and Montefiore teams to develop, modernize and support the application.
- ASP.NET Core Razor Pages, ASP.NET MVC, Entity Framework and SQL Server on the backend; Angular and React SPAs on the front end.
- Translated business analyst user stories into technical specifications, API contracts and validation rules.
- Delivered through Git/GitHub, Azure DevOps and GitHub Actions pipelines.
